They make it possible for members to get the right health care treatment for their needs and for Aetna to keep its competitive edge. Aetnas Medicare Advantage Program is growing in Blue Bell, PA. We are currently seeking Nurses who will provide our Medicare Members with more information, more choice and more control over what matters to them most - their health and well being. To succeed, we need talented, dedicated, diverse people who are passionate about wanting to make a meaningful difference in the lives of our Medicare Members. RESPONSIBILITIES -Assesses targeted Aetna Medicare member health care needs including identifying and addressing gaps in care -Coordinates a case management plan in collaboration with the member, members family and interdisciplinary team and performing regular evaluations of the members progress towards expected outcomes -Focuses on coordinating cost effective sequencing of care, returning members to optimal level of health and maximum level of independence and optimal level of function, and increasing member and provider satisfaction through a collaborative plan of care -Identifies needs for intervention and to impact quality of life -Assesses appropriate care setting and ancillary resources within benefit parameters -Collects clinical information from a variety of sources, and collaborate effectively with those involved in the case REQUIRED SKILLS: -Registered Nurse with current active with unrestricted PA license -Minimum of 3-5 years of recent clinical experience -Experience in geriatric case management, home care, and or subacute nursing preferred. Current Case Management certification a plus -Good time management and organizational skills along with the ability to manage continually changes priorities are needed -Candidate must possess computer and keyboarding skills -Communication between the member and all members of the healthcare team paramount throughout the case management process -Excellent written and verbal communication skills -Knowledge of Excel and Word -Managed Care experience a plus
